Center For Family Services is seeking an experienced and dedicated Case Manager for the Keeping Families Together program. The Case Manager provides direct services to a caseload of 15-19 families utilizing the KFT model. This individual provides in-home case management services to meet the family's goals which may include parenting skills, household management, communication skills, child development, basic budgeting, and establishing daily routines. This is a temporary position with the possibility for long-term opportunities.
Location: Camden County, NJDuties and Responsibilities:
The Case Manager provides linkage to community resources including child care, educational needs, food banks, and WIC and also connects families and youth to Family Success Centers, sports and recreational activities, creative arts, and educational programs. This individual meets regularly with supervisor for individual and group supervision for case review, staff development, and program development and also completes necessary paperwork within outlined time requirements and in a professional manner. The Case Manager has excellent communication, writing and computer skills. The Case Manager acts as a support for the parents, assists with any transitional issues that occur in the process of reunification, and provides a support system to families in recovery seeking to improve their overall mental and physical health. This individual transports individuals and families to appointments, meetings and activities which are part of their Family CARE plan and demonstrates for families how to advocate for themselves and their children within the community to help establish self-sufficiency.
