Position Summary:

Center For Family Services is seeking a Lead Program Coordinator for our ReConnections Program. ReConnections is a program that offers peer-to-peer coaching and support services to family, friends, and loved ones of individuals struggling with addiction. The Lead Program Coordinator will be responsible for providing outreach and coordination of services with all systems involved with the families.

Duties and Responsibilities including but not limited to;

┬ĚThe Coordinator oversees  four (4) full time equivalent staff in providing peer-to-peer compassionate support primarily in person, virtually, when deemed appropriate by supervisor, and by telephone or email. Services include:

    • Developing an Individualized Wellness Plan with the family member(s) that is culturally competent with relevant services and identifies goals with measurable objectives the family member wishes to achieve.
    • Offering resources to family members seeking information on substance use disorder and behavioral treatment.
    • Providing outreach and coordination of services with all systems involved with the family (i.e. support community, Mental Health and Substance Use Treatment community, IME, and Opioid Overdose Recovery Programs (OORPs), SupportTeams for Addiction Recovery (STARs), Department of Children and Families, Drug Court, Probation)
    • Informing family members of loved ones at risk of an Opioid Overdose about Naloxone training and where Naloxone kits can be obtained free or purchased.
    • Connecting families with other family members if desired.
    • Advocating on behalf of families.
    • Providing lists of 12 Step and Recovery Support Group Meeting locations and/or other Mutual Support Groups for families.
    • Guiding family members in exploring effective coping strategies such as self-care.
    • Facilitating psychoeducation and support group meetings as needed.
    • Offering supportive strategies on how to motivate their loved one to enter and stay in treatment and connect with recovery support services
    • Teaching positive communication skills
    • Creating an engaging and respectful environment that recognizes, supports, and encourages family members as important collaborators in their loved one's care.
    • Informing families of "patient brokering" that may result in unethical practices during their time of need to seek immediate treatment for their loved ones

Qualifications

  • The FSCC shall possess a minimum of a high school diploma or equivalency and will provide peer-to-peer compassionate support to program participants.
  • Applicants must have the ability to multi-task
  • Excellent Communication and Organizational skills
  • Current New Jersey driver's license with a driving record of at least three years
