Duties and Responsibilities:
Evaluate, diagnose and provide treatment to Center clients:
Becomes trained in medication assisted treatment and telepsychiatry and provides both services to CFS clients.
Develop comprehensive holistic behavioral health assessments for children, adolescents, adults, older adults and clients with behavioral health disorders; involve family members as indicated.
Develop individual and family focused treatment plans which are goal directed; involve family members as indicated; treatment plans will include management of physical and mental health problems; re-evaluate plans as needed and at intervals specified in Center procedures.
Prescribe medication in accordance with his/her license and the Center's formulary; this may include prescribing controlled dangerous substances and following government regulations and agency policies.
Provide medication evaluation and management services including medication counseling and education, psychoeducation to assigned clients and family caregivers.
Order and evaluate diagnostic tests.
Collect vital signs such as height, weight and blood pressure.
Coordinate treatment with other Center For Family Services and staff.
Coordinate treatment planning with other medical practitioners and residential and other behavioral health treatment providers.
Complete psychiatric evaluations and treatment progress notes in accordance with agency policies and procedures.
Administer injectable medications as prescribed.
Manage medication prior authorizations when necessary.
Work cooperatively with agency medical staff, including back-up coverage and medication renewal requests as needed.
Be familiar with community resources and refer clients as indicated.
Record keeping within Center's electronic health record system
Adhere to Center policies and procedures regarding sample medication inventory.
Assist medical staff in the development and implementation of performance improvement activities; oversee record reviews and participate in other performance improvement activities as needed.
As the Nurse Manager at CFS, assumes day-to-day responsibility for the operations of the medical team, in the Medical Director's absence, such as handling client/clinical staff concerns, acting as a liaison between medical staff and internal/external stakeholders and fills in-seeing other provider's clients--when other medical staff member is unexpectedly out.
Provide guidance and supervision to other CFS Warren County based APRN staff.
Provide direct supervision to part time Medical Assistant. The Medical Assistant supports the medical staff by assisting with client vital signs, administration of injectable medications, insurance company prior authorizations for medications and obtaining hospital discharge records for clients.
With oversight from the Medical Director, directs the medication assisted treatment (MAT) and the tele-psychiatry program.
Provide education on psychiatric and MAT medications to agency clinical staff.
Assume responsibility for professional development, meeting licensure requirements, and maintaining credentials through continuing education activities.
Maintain a current collaborating agreement with a Center staff psychiatrist (or designee) who is readily available at all times through electronic communications, for the purpose of consultation; meet face to face on a regular basis with collaborating physician for case review.
Other responsibilities as assigned.
REQUIREMENTS:
Master's degree in Psychiatric Mental Health Nursing from an accredited university
ANA clinical nurse specialist certification
Current New Jersey License for RN and APN
