POSITION SUMMARY:

The Care Coordinator/Credible Messenger will provide indirect and direct case management services to youth and families pre and post-release. They will oversee and monitor program implementation for the identified grant provider. The role will provide support, guidance, and coordination of services to youth and families referred to the program. The Care Coordinator/Credible Messenger will facilitate groups, program activities, supportive counseling, community outreach, intensive case coordination services, and other program functions to meet the program's needs. The Care Coordinator/Credible Messenger ensures program goals are on target, clients are accessing treatment/services, and identified service goals. 

Duties and Responsibilities: 

  • Provide Pre/Post-release support with active counseling, referral services, intensive case management, treatment planning, goal monitoring, group counseling, mentoring, follow-up services, advocacy, and support services to re-entry youths and families. 

  • Maintain fidelity to evidence-based and evidence-informed interventions used in Project Reconnect. 

  • Complete documentation and other reporting requirements on a timely basis. 

  • Perform specialized activities and functions specific to the program. 

  • Meets regularly with supervisor and participates in agency collaboration team meetings. 

  • Meets with the treatment team weekly to share contacts/outcomes, and ensure that all clients are appropriately supported. 

  • Contributes to agency committees and unique tasks, promotes a sense of agency with coworkers and staff, and communicates in ways that support the agency mission and values, while adhering to policies and procedures. 

  • Carry a caseload of up to 15 individuals at any time. 

  • Provide comprehensive case management assessment that addresses life domains such as housing, finances, transportation, legal services, education, employment, health care, and family strengths/needs. 

  • Provide guidance and support to the youth and families to effectively connect individuals to available treatment and services in the community, including substance use disorder treatment. 

  • Develop a person-centered Integrated Family Case Plan with strategies for success, identifies priorities, desired outcomes, and methods and resources to obtain outcomes based on the case management assessment. 

  • Utilize the case management system to assist youth and families with recording engagement and interactions with program participants and track key program goals, objectives, and outcomes. 

  • Provide case monitor support to youth and family 90 days post-discharge. 

  • Coordinate needed interventions and services provided by multiple agencies 

  • Use engagement skills to motivate and encourage individuals to voluntarily access services that would assist them in maintaining a healthy lifestyle 

  • Provide training to clients on recognizing relapse triggers and strategies to avoid recidivism 


REQUIREMENTS: 

  • Bachelor's degree in a Human Services related field with a minimum of 2 years of related work experience 

  • Individuals with a history of gang membership or criminal background must have made a change in their life and demonstrate that they have turned away from crime 

  • Must be culturally appropriate by living and/or have knowledge of Camden County communities and known to high-risk individuals 

  • Knowledgeable of formal/informal community systems and resources 

  • Experience working with at-risk youth 

  • Excellent communication skills and computer skills (i.e., spreadsheet/database application for data and reporting). 

  • Valid driver's license, insurance, and good driving record
