Position Summary: 

The Expanded Medically Assisted Treatment for Inmates (EMATI)-Salem MAT Case Manager is responsible for assessing client needs, developing individualized treatment plans, and connecting individuals to essential resources for recovery. This position requires collaboration with Peer Recovery Specialists, program supervisors, and community partners to ensure a comprehensive support system for clients. The Case Manager plays a crucial role in advocating for clients and ensuring they receive the necessary support for successful reintegration and sustained recovery.

Duties and Responsibilities:

- Review, complete, maintain, and monitor all required documentation within established timeframes in accordance with Federal, State, County, accreditation, and agency regulations, policies, and procedures.

- Complete comprehensive client intakes and case management assessments addressing life domains such as transportation, legal services, health care, and family strengths/needs.

- Assess client MAT needs and identify barriers to recovery, ensuring all relevant information is documented accurately.

- Develop person-centered integrated MAT plans that outline priorities, desired outcomes, and strategies for recovery and connection to MAT services post-release.

- Collaborate with clients to establish goals and link them to appropriate resources based on case management assessments. - Connect clients with community resources to address specialized needs, such as HIV/AIDS services, mental health support, chronic/acute health care, legal or criminal justice-related assistance as well as MAT services.

- Access and Facilitate access to substance use disorder treatment and Medication-Assisted Treatment (MAT) when necessary.

- Monitor and ensure accurate documentation of services, utilization, and outcomes in systems such as AWARDS, DocuSign, and/or any other electronic technology system.

- Collaborate with Peer Recovery Specialists on linking, advocating, and referring clients to appropriate recovery services. - Collaborate with Program Supervisors on recruitment, retention, and HR-related activities.

- Complete thorough and comprehensive case reviews according to the Lifeline Policy and Procedure manual. - Support program leadership in recording, gathering, researching, and documenting program data for daily, monthly, and yearly reports as required.

- Support program leadership in evaluating, training, promoting, and guiding staff within approved agency guidelines. - Participate in designated agency-wide committees, such as Trauma-Informed Care (TIC), attend individual and group supervisions, and attend all required meetings.

- Deliver services in a culturally competent and linguistically appropriate manner, ensuring alignment with program and client needs.

    REQUIREMENTS:

    • Bachelors or Master's Degree in Health, Psychology, Counseling, Social Work, Education, or other behavioral health profession, CDAC, or, LCDAC
    • Three years' experience in Substance Use Disorder services
    • Shall possess knowledge in formal/informal community systems and resources.
    • Must possess the knowledge, skills and experience necessary to perform competently case management activities. 
    • Must have at least three (3) years' experience working with high need individuals involved with substance use disorders.
    • Shall demonstrate evidence of working with substance use populations or evidence of addiction coursework. 
    • Requires car, valid registration, insurance and driver's license and must maintain compliance with CFS Motor Vehicle policies
