Location: Mercer County, NJ
Job Type/Status: Full-Time
Duties and Responsibilities:
Case management of 4 Mary's Place residents.
Attends required meetings and conferences.
Enters data in AWARDS and other required databases.
Conducts interviews for potential residents.
Conducts life skills assessments according to the Youth Thrive Curriculum.
Conducts onsite meetings with residents of Mary's Place.
Assist with teaching independent living skills, job readiness and other needed skills.
Follows the Nurtured Heart Approach tools when interacting with residents and staff.
Provide direct counseling and other direct service activity.
Assures the continuing compliance of training and development as required by CFS.
Coordinates program activities with appropriate liaison agencies to ensure constructive collaboration.
Submits reports on time to DCF and DCPP team members.
Updates the Adolescent Housing Hub.
Develops and promotes relationships with other social service agencies and may represent the program and agency in the community.
May serve on Agency-wide committees.
REQUIREMENTS:
Bachelor's degree
At least one year's experience working with program population, preferably in a residential setting
Case Management experience
Experience working with youth or young adults
