Salary: $48,000 to $50,000
Position Summary:
The Case Manager provides screening, assessment, advocacy and ongoing case management for homeless families placed in Shelter for those that are transitioning back to the community. The Case Manager assist with assessments, discussion of goals, establish a service plan to meet the goals, follow up, provide advocacy, modeling and tools needed to overcome barriers. Documentation on all interaction with details, referrals, outcomes and next step.
Duties and Responsibilities:
- Provide intake, screening, assessments, shelter housing placement paperwork and ongoing monitoring for candidates.
- Provide participant intake, address immediate needs, assessments, and ongoing goal setting and monitoring.
- Assist participants to establish stable housing by helping them develop problem solving and goal setting skills. Connect with resources for housing: VISPDAT and Tenant Ready Curriculum
- Assist participants in determining their needs in all areas, including, but not limited to employment, education and financial needs.
- Provide information/ referral ensuring warm transfer: calling program first, a three way conversation ensuring other program understand the needs and participant understand what assistance they will be receiving.
- Maintain accurate and up to date participant's records and case notes.
- Develop and maintain good relations with participants and other community resource providers.
- Assist with identification of and facilitation of shelter maintenance needs including arranging the cleaning of the units as needed.
- Facilitate harmonious and appropriate community or group living situations as necessary.
- Assure that participants maintain their housing units in a safe and healthy condition.
- Serve as mentor/ advocate for needed services.
- Proven ability to be flexible and to work in a fast changing environment.
- Complete reports as needed
- Assist with duties as needed to ensure success of participants and program
- Participate in regular supervision and meeting. Contacting supervisor as needed for support in role as case manager. Cross train working toward professional development.
- Represent the MCR program and CFS agency services at external meetings to further the positive image of CFS in the community.
- Understand and adhere to CFS policies around confidentiality and all standards of conduct as described in the CFS/MCR/COA Manual. Understand procedures for emergency and complete incident reports.
- Understand time off and other procedures that effect the day-to-day operation of the program.
Requirements:
- A Bachelor's Degree in Social Work, Psychology or Human Services is mandatory.
- Experience working with people, especially on a one-to-one basis, and demonstrated sensitivity toward the needs of vulnerable population
- Excellent communication and organizational skills with basic computer knowledge
- Ability to work independently, accurately and efficiently, with an eye for detail
- Excellent phone manners and customer service skills
- Creates and executes CFS/MCR objectives, meeting CFS/MCR mission and commitments. Reflects CFS/MCR values and philosophy in all business interactions.
