Position Status: Full Time
Position Summary:
The SOS Case Manager is responsible for canvassing and mapping street outreach services, and providing case management to homeless youth. Contributes to and serves on internal and external committees and special tasks, and promotes a sense of agency with co-workers and staff, and communicates in ways that support agency mission and values.
Duties and Responsibilities:
- Understand and relate to the needs of clients with diverse ethnic, cultural, educational, and socioeconomic backgrounds.
- Knowledge of target areas and willingness to venture into areas frequented by homeless youth.
- Work effectively with at-risk youth and families, colleagues and service providers.
- Work a flexible schedule, including nights and weekends; on-call 24/7 for hotline calls/referrals.
- Conduct Intake and Immediate Needs Assessments and monitor clients progress.
- Provide psycho-educational counseling, develop and implement strategies to raise awareness and to educate youth about sexual and domestic violence, and human trafficking.
- Assist youth in securing social services, education, safe housing and job-related opportunities.
- Provide emergency basic need and transportation services to homeless youth.
- Provide case management/referrals services to include coordination of visits at shelter facilities.
- Complete required documentation for client participation and internal record keeping and reporting in the EHS-AWARDS database in an accurate, consistent and timely manner.
- Participate in community collaborations and events to identify additional programming and resources appropriate for client needs.
- Represent the agency in the community, developing vital service linkages and partnerships and providing education through the disbursement of materials and public speaking engagements.
- Adhere to goals as written within S.O.S. grants.
- Complete monthly and quarterly reports as required in a timely and consistent manner.
- Provide community education to schools, churches, and other organizations.
- Attend trainings and seminars for ongoing professional development.
- Adhere to CFS agency policies and procedures.
Requirements:
- The SOS Case Manager must successfully complete the 60-Hour New Jersey State Confidential Sexual Violence Advocate training, Domestic Violence and Human Trafficking Training provided by Services Empowering Rights of Victims.
- Knowledgeable about working with at risk youth, runaway and homeless youth, diverse cultures and communities, intersectionality, social and other indicators of substance use, mental health and holistic well-being.
- Excellent written and oral communication skills; Experience with public speaking, delivering educational trainings and facilitating group activities.
- This position requires an Associate Degree in Human Services, Psychology, Social Work or a related field; college credits and/or 1-2 years of relevant work experience may be considered in lieu of degree.
