Job Type/Status: Full Time

Salary: $45,000 - $46,000 per year

Program Summary:

The Community Health Care Worker will be working with older adults in-home/in community and link clients to each other by creating opportunities for social activities. The Community Health Care Worker will provide health education and help individuals overcome barriers to health goals. Where possible, the community health care worker will create means for otherwise homebound clients to be able to meet with others in-person, or in the community. Our candidate will have the ability to problem solve and work independently. Excellent computer and organization skills are a must

Duties and Responsibilities:

  • Experience working with the older adult community
  • Extensive knowledge of Warren County communities and activities/events
  • Coordinate interventions and services provided by multiple agencies, including within CFS programs
  • May provide specialized activities and functions specific to program
  • Participate in continuing professional development training and continuing education
  • Sensitivity in functioning in a multicultural work setting and an understanding in human relations
  • Possesses the ability to effectively use technological tools and platforms in order to complete their work functions efficiently. Including, but not limited to, proficiency in: Care logic, Microsoft Windows and corporate network resources, required software and systems that are pertinent to the position, Gmail and additional web-based email services, and the internet.
  • Conducts self as a role model in interactions with staff, colleagues, and funding sources in accordance with Center For Family Services' Code of Ethics
  • Excellent organizational and time management skills in order to manage multiple demands
  • Participates in provided trainings to increase knowledge and awareness on best practices in Trauma Informed Care delivery modalities
  • Utilize Trauma Informed Care methods when engaging with staff, clients, and stakeholders in the community
  • Complete required documentation within established timeframes per Federal, State, County, and agency regulations, policy and procedure.
  • Appropriately utilizes supervision process and agency supports
  • Participate in team meetings to provide updates, brainstorm ideas, plan events, receive input and support from the Program Director
  • Collects statistics and completes written reports as required

Job Requirements (Qualifications):

  • Proficiency in Spanish or another language is a plus
  • High School Diploma or GED
  • Previous experience in health education or community outreach
  • Strong knowledge of community resources, services, and challenges facing the
  • Excellent communication and interpersonal skills
  • Strong organizational and problem solving skills
  • Ability to work independently or part of a team
  • Basic computer skills, including familiarity with Microsoft systems
  • Valid driver's license and reliable transportation
  • Comfortable working in various settings, including the in-home setting
  • Demonstrated commitment to improving community health
  • Cultural competency and sensitivity
