Position Summary:
Facilitates/co-facilitates home visits with clinician for intensive, in-home clinical treatment program for families with young children at risk for abuse, neglect, poor developmental outcomes and removal due to parental substance use. The overarching goal of the program is to promote stability, safety and permanence for families. Works with a caseload of 12 families.

Duties and Responsibilities:
Responsible for establishing and maintaining a regular and long-term relationship with the family
Assist the family in establishing goals and a plan for the accomplishment of those goals
Family-centered and strength based activity will occur primarily within the participant's home
Assess of the normal growth and development of the target child
Attend weekly social club meetings (required to work one evening per week)
Assist in strengthening the parent-child relationship
Assist in increasing the family support system and increase the family's ability to problem solve and assume the role of advocating for themselves and their children.
Assist clients in school and job preparation
Provide linkages to community resources
Maintain contact with the client's MAT provider, as appropriate
Assist clients in applying for NJ family care, as needed
Connect clients to a medical home
Refer children to early intervention services, as needed
Utilize abstinence-related tracking tools to graph recovery-related activities
Work in close collaboration with DCP&P local office from the time of the referral until the client is discharged
Attend required trainings
Enters and maintains data entry of client information info database(s)
Travel throughout New Jersey to attend required core trainings, curricula training and ongoing trainings required by the model and/or agency
Participate in technical assistance meetings and calls
Submit all required data to the Clinical Supervisor
REQUIREMENTS:
Minimum of a Bachelor's Degree in a related human services field (social work, psychology, sociology, etc.)
Experience working in a behavioral health related field, knowledge of effects of substance use disorders on families a plus
Bilingual preferred
Knowledge of normal child growth/ development and parent - child relationships
Experience working with community agencies, including implementing referral processes.
Experience working with culturally diverse communities/families and the ability to be culturally sensitive and appropriate
Strong writing, effective communication and organizational skills
Infant mental health endorsement level I or II preferred
