Position Summary:

Center for Family Services is seeking a full-time case manager for our Children's Partial Care program providing services to clients ages 13-17.

Location: Camden, NJ

Job Type/Status: Full Time, weekdays from 1:00 PM to 9 PM, summer program weekdays from 9 AM to 6 PM

Duties and Responsibilities:

  • Complete required documentation within established timeframes per Federal, State, County, and agency regulations, policy and procedure.
  • Adhere to HIPAA regulations in all related member matters.
  •  Provide direct service to caseload of clients that includes: facilitate group programming through evidence based curriculums, provide individual and/or family supportive counseling, and provide services such as referral, advocacy, and linkages.
  • Participate in the development of treatment plans, comprehensive assessments, discharge planning, case conferences, and treatment plan meetings.
  • Participate in the development and planning of daily group therapeutic activities.
  • Provide transportation of clients to and from program on a daily basis.
  • Work collaboratively with other team members to ensure facilitation of programming.
  • Maintain contact with families and collaterals on a minimum of a monthly basis.
  • Participate and communicate in team meetings regarding clients behaviors and ongoing treatment issues
  • To carry out designated duties as assigned by supervisor.

REQUIREMENTS:

Bachelor of Social Work degree or comparable Bachelor of Arts degree in Human Services, Sociology/Psychology from an accredited institution.

  • One (1) year of experience in the provision of mental health services to youth.
  • Basic interviewing, assessment, and case management skills.
  • Initiative and creativity.
  • Dependability and ability to work well with others.
  • Sensitivity in functioning in a multi-cultural work setting.
  • Ability to communicate clearly and consistently.
  • Ability to complete documentation independently, accurately, and adhere to time frames.
  • A valid NJ driver's license.
  • Understanding of Trauma Informed Care practice and treatment.
  • Proficient in use of computer, familiar with use of an electronic health system.
