Salary: $58,000 per year
Position Summary:
Lead Case Manager is responsible for coordinating case management and safe and timely release services, supervises care management staff and practices. Assesses the needs of unaccompanied children in care, develops Individual Service Plans, screens for human trafficking concerns, facilitates the safe and timely release or discharge of children and youth, documents the provision of services in case files.
Duties and Responsibilities:
- Maintains a caseload of up to 2 UAC youth and works on the reunification of each youth with a qualified sponsor.
- Supervises 6 Case managers ensuring a one to eight UAC ratio and compliance with CM duties.
- Ensure compliance with ORR Data Base and sponsor related approvals.
- Implement and administer a case management system that tracks and monitors a UC's progress on a regular basis to ensure that each UC receives the full range of program services in an integrated and comprehensive manner.
- Coordinates and provides supervision to case managers to individual support required to meet with assigned UC, at a minimum, once a week.
- Ensures completion of steps required to safely release UC to appropriate sponsors promptly and, on average, within 30 days of referral. This includes legal, medical and trauma informed evidenced based practices.
- Designs Sponsor reunification plan for each youth assigned to caseload at the onset of the youth's shelter placement, and update the plan on a weekly basis
- Performs direct counseling services for assigned caseload of residents in coordination with the Clinical and Administrative Juntos team.
- Transports youth to various appointments including medical, recreational, educational, and employment appointments.
- Performs case management for youth assigned to caseload and assists in other advocacy for sheltered youth as needed.
- Performs other crisis intervention, counseling, youth supervision and other related activities as necessary to meet 24-hour operational activities of shelter program.
- Advocates for youth with the court system, local and state agencies, ORR, family members, etc.
- Does progress notes, collects statistics and completes written reports as required. case management team is also responsible for maintenance of the ORR/DCS database, UC Portal. All placement, transfer, and release requests, as well as all significant incident reports are processed through this web-based system.
- Meets with the designated program supervisor and clinical supervisor regularly for time structuring and case planning. When necessary, case management staff are required to also work collaboratively with agencies that conduct home studies and provide post release services. Care providers are required to conduct safety and well-being follow-up calls to a UC and sponsor 30 days after the UC is released.
- Attends training as needed & / or required to improve and maintain clinical and crisis intervention skills.
- Assist in maintaining 24-hour case management coverage and provides rotating on-call emergency coverage as needed.
- Performs other duties as required.
Requirements:
- Certified Social Worker (CSW) or equivalent certification, if applicable.
- Master's degree in the behavioral sciences, human services or social services fields;
- Or bachelor's degree and at least three years' progressive employment experience that demonstrates supervisory and case management experience.
- Bilingual language fluency in both English and Spanish
- Excellent Driving skills and clean driving record
- Be a licensed driver for at least 3 years
