Position Summary:
The Licensed Practical Nurse (LPN) coordinates, maintains, and updates the medication management system under the direct supervision of the Registered Nurse Manager, to allow for the administration of medication to UAC in accordance with HHSC, ORR, and DHHS policies and procedures.
Duties and Responsibilities
Communicates and collaborates with the RN to obtain technical assistance in the implementation of medical services. - Completion of TAR's and TB supplemental.
Collaborate with outside providers to attain UAC medical records, vaccination, lab results, etc to assure all medical files are up to date in a timely fashion.
Coordination of the provision of initial physicals by a licensed physician, - Preparation and revision of medication logs,
Timely reporting of medical incidents (e.g., SIRs, HHSC reports)
Preparation of immunization records for County Health Department audits, Office of Refugee Resettlement audits
Ensures services are expedited by ensuring a physical screening is provided to UAC.
Maintains relationships with local providers, pharmacies, as well as with ORR and DHHS.
The LPN prepares and reviews statistical information for weekly, monthly, quarterly, and annual reports.
Arrange or partners with other health professionals regarding health and safety standards for out of home childcare and childcare licensing requirements
Follows disease-reporting requirements for care providers, immunizations for children and injury prevention for children.
Connects with community health and mental health resources for children
Serves on the treatment team with therapists, case managers, and other designated staff.
Ensures health and safety of all residents including provision of initial physical examination next working day, scheduling follow-up physical examinations by a physician within ten days and obtaining the health history of all new admissions.
Administers physical examination, treatment or referral of all emergent health concerns of residents in accordance with policies and procedures and standards of health care for nurses.
Provides life skills education in the areas of nutrition, hygiene, sexually transmitted diseases, infection control, and other health related topics.
Provides individual counseling on health-related issues with identified youth.
Coordinates appointments with medical and psychiatric consultants, ensures all over the counter and prescription supplies and medication are available and correctly administered.
Explains medications to both residents and staff, such as side effects, reasons for the medications, etc.
Prepares and implements a comprehensive health plan to ensure that each resident's medical, dental, and other health needs are being met adequately and promptly.
Completes and monitors Medication Log with necessary information.
Administers appropriate medications to residents-only prescription medication when authorized by a physician.
Ensures that the First Aid Kit and medication cabinet is fully stocked at all times with necessary medical supplies.
Disposes of old medications, according to the facility's policies and procedures. -
Provides UAC's and staff health groups and trainings as required for program need and conducts 1 group per week for UAC's.
Ensures that all UAC's are discharged with prescribed medication according to facility's policies and procedures.
Maintain medical files and assure that all medical documentation for each UAC is filed in case files and timely and accurate upload to the program data system.
Qualifications
Minimum of Associate's degree in Nursing
One to two years, with one year specifically working within child welfare with children and/or adolescents in a social services setting.
Bilingual in Spanish/English writing and speaking.
Preferred
Bachelor's degree in Nursing (BSN)
Licensed as a Practical Nurse in the state of New Jersey for 2 or more years
Five years, with one year specifically working within child welfare with children and/or adolescents in a social services setting. Preferred experience with undocumented children and families.
