POSITION SUMMARY:
Supervises the Keeping Families Together program designed to provide housing and support to at risk children and their families. Responsible for smooth operation of program, all reports, employee issues and maintaining relationships with community providers
Duties and Responsibilities:
Reports to Nursing Supervisor and Medical Director (if nursing supervisor is absent or an emergent issue) on daily needs in their current progress of minors medical clearance.
Arrange or partners with other health professionals regarding health and safety standards for out-of-home child care, child care licensing requirements, disease reporting requirements for care providers, immunizations for children, injury prevention for children, medication management and knowledge of community health and mental health resources for children.
Demonstrates a basic knowledge of CFS' continuum of services and provides appropriate referral when applicable and needed.
Provide skilled treatments (vital signs, PPD reads, vision screening, and additional support with providers on appointments) to assigned minors. Trainings will be provided to meet the needs of minors in program.
Expect to be scheduled for a full 7 days with a 24 hours oncall rotating schedule
Administer medications by following six rights of medication administration per program standards.
Interacts and works with children, adults, and families served, as well as colleagues, and/or community members in a non-biased, inclusive, and respectful way regardless of cultural differences and circumstances.
Appropriately utilizes supervision process and Agency support in order to best execute assignments, duties, and responsibilities.

Report to Nursing Supervisor and Medical Director on daily needs in their current progress of minors medical clearance.

Responsible for the necessary training to ensure competencies and professional development of program/team staff.

Reports any suspected child/elder abuse and neglect per New Jersey's guidelines.
Conducts self as a role model in interactions with staff, colleagues, and funding sources in accordance with Center for Family Services' Code of Ethics.
Works in a professional manner with appropriate administrative staff in getting administrative information to agency managers.
Maintains appropriate and professional relationship with all external parties including, but not limited to, vendors, funders, partners, state and federal agencies.
Maintains program compliance with all funding and licensing procedures and requirements.
Discusses personal and professional staff issues, when necessary, only with appropriate individuals.
Respects confidentiality of clients and staff by adhering to the Code of Ethics and Confidentiality Agreement.
Coordinates with CFS Operations Department on facility improvement, ensuring that each site complies with CFS standards of excellence - ensures that systems are in place for required and enhanced cleanliness, safety and homelike environments.
Assists the treatment team to provide the utmost care by ensuring the health and safety of all youth in the program.
Assists the treatment team including leadership, clinicians, case management, medical, Quality Improvement, and residential staff in collaborative efforts to provide a plan of care and education in health-related topics based on each minors individual requirements.
Helps prepare and implement a comprehensive plan of care to ensure that each minor's medical, dental and other health needs are being met adequately and properly executed.
Provides aid during medical appointments to ensure that all requirements for documentation are adhered to and to coordinate the appointments with medical, specialists, dental and psychiatric consultants.
Assisting with medical appointments at the CFFS- EHT clinic which will include, but not limited to, obtaining vitals, assisting the provider and administering vaccines.
Manages medical appointments with providers and / or provider partners and ensures calendar invites are sent out with confirmation of appointments which include and not limited to the following Initial Medical Exams (IME)s, Lab Corporation, Trident Care, and any other specialist referrals requested by providers.
Additional appointments outside of IMEs by providers will need to be discussed with the Medical Director and nursing supervisors to determine priority of scheduling within our program or upon reunification with sponsors.
Ensures physical examination Treatment Authorization Request (TAR) referral of all emergent health concerns of minors are completed on the same day of request.
Ensures all documentation is completed and uploaded to UAC portal within medical department time frames policy.
Accurately and consistently completes medication cabinets, check expiration of medication, and updates or minor medication logs.
Reviews and administers appropriate medications based on the Over The Counter (OTC) orders and when authorized by the Medical Director and/or physician.
Assist with disposal of old medications properly.
Assists the program in transporting minors to appointments via telehealth and/or in person.
Adheres and educates minors on all Federal and States and VFC immunization requirements including Covid-19 vaccination and Flu (during season).
Prepares and maintains copies of original documentation of minors medical file from admission to discharge.
May be required to cover shifts while a minor is hospitalized within our program. Medical will be able to bring their lap top and work issued phone to monitor their sites needs or discuss with Medical team plan of care for ones cover shift.
REQUIREMENTS:
Associates Degree in Healthcare and/or related field required
Current CPR card for Healthcare Professionals (CPR/AED/First Aid).
Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) or National Certified Medical Assistant (NCMA) or Clinical Medical Assistant Certification (CMAC) or Nationally Registered Certified Medical Assistant (NRCMA) or Certified Clinical Medical Assistant (CCMA).
Bachelor's degree in Healthcare and/ or relevant field prferred
3-year employment experience in the healthcare setting with children and/or adolescents in a social service setting.
Bilingual: English/Spanish required
