Job Type/Status: Full Time, Monday-Friday, 11am-7pm
Salary: $48,000 per year
Position Summary:
The Lifeline Patient Navigator is responsible for connecting individuals with substance use and mental health disorders to appropriate care and recovery resources. They coordinate services, ensure compliance with policies, and support program leadership through documentation and data management. Working closely with clients, healthcare providers, and peer specialists, they promote continuity of care and advocate for client needs.
Duties and Responsibilities:
- Ensure compliance with funding requirements, contracts, and Lifeline Policy and Procedure manual.
- Provide evidence-based comprehensive case management assessment that includes life domains such as housing, finances, transportation, legal services, vocational, employment, healthcare, and family strength/needs.
- Ensure timely program response to client and hospital needs and coordination of care with system partners including but not limited to treatment facilities, recovery support services, and any other social service.
- Develop linkage and link clients to resources that address specialized needs, such as services related to HIV/AIDS, mental health disorders, chronic and acute health problems, pregnant and parenting women, veterans, and problems stemming from involvement in the criminal justice system.
- Review, complete, maintain and monitor all required documentation within established timeframes in accordance with Federal, State, County, accreditation, and agency regulations, policies, and procedures.
- Communicate with the DMHAS Interim Managing Entity (IME) for substance use disorder assessment authorization and subsequent treatment, as needed.
- Develop an integrated case plan that is consumer-centered and includes strategies for recovery. The plan shall identify priorities, desired outcomes, and the strategies and resources to be used in obtaining outcomes based on the case management assessment.
- Monitor and ensure accurate documentation of services, utilization, and outcomes in systems such as AWARDS, DocuSign, and/or any other electronic technology system.
- Collaborate with Peer Recovery Specialists on linking, advocating, and referring clients to appropriate substance abuse treatment and recovery services as well as provide a warm hand-off to appropriate treatment and recovery services to include referrals to DHS licensed programs that offer Medication Assisted Treatment (MAT).
- Collaborate with Program Supervisors on recruitment, retention, and HR-related activities.
- Complete thorough and comprehensive case reviews according to the Lifeline Policy and Procedure manual.
- Support program leadership in recording, gathering, researching, and documenting program data for daily, monthly, and yearly reports as required.
- Advocate for appropriate and effective community treatment and recovery as well as empower clients to make self-determined and self-directed choices about their recovery.
- Support program leadership in evaluating, training, promoting, and guiding staff within approved agency guidelines.
- Participate in designated agency-wide committees, such as Trauma-Informed Care (TIC), attend individual and group supervisions, and attend all required meetings.
- Deliver services in a culturally competent and linguistically appropriate manner, ensuring alignment with program and client needs.
Qualifications
- Bachelor's level degree in Human Services Field: Psychology, Social Work, Counseling, or a related field minimum two years of program experience within a behavioral health setting
- Minimum of 3 years of program experience working with individuals with substance use and mental health disorders
- Knowledge and experience in full complement of evidence-based and administrative practice to include Trauma Informed Care Principles and an understanding of harm reduction
- Proven ability to constructively manage conflict and solve problems
- Knowledge and competency in the utilization of Electronic Health Records
- Exemplary writing and speaking skills
- Federal and State regulations require employees in this position to be fully vaccinated.
