Job Type/Status: Full-Time
Duties and Responsibilities:
Complaint with funding source, contract and or program's Annex A
Complete required documentation within established timeframes per Federal, State, County, and agency regulations, policy and procedure.
Provide care management: facilitate group programming through evidence-based curriculums, provide individual and/or family supportive counseling, and provide referrals, advocacy, and linkages
Create and provide educational presentations to family members of individuals at all stages of recovery. These presentations will provide education about the illness of addiction, attendees will learn coping skills, linkage and referral to resources and supportive behaviors. This program will utilize the evidence-based curriculum of CRAFT.
Participate in the development of service plans, comprehensive assessments, discharge planning, and case conferences
Coordinate interventions and services provided by multiple agencies, including within CFS programs
Appropriately utilizes supervision process and agency supports
Participate in team meetings regarding the individual's behaviors and ongoing service needs
Collects statistics and completes written reports as required
Administer pre and post tests at the start and end of program engagement
Actively work to reduce the stigma associated with a substance use disorder in the community
May provide specialized activities and functions specific to program
Conduct outreach and presentations within the Hudson County community to inform the public and community partners such as agencies, schools and places of worship of the services.
Facilitate stationhouse adjustment curriculum with at risk and juvenile involved youth through case management, restorative justice groups sessions and group activities utilizing the STRIVE curriuculum
REQUIREMENTS:
Bachelor of Social Work degree or comparable Bachelor of Arts degree in Human Services, Sociology/Psychology from an accredited institution. Experience may substitute for education on a year for year basis
Experience in care management
Strong interpersonal skills
