Salary: $65,000 to $70,000 per year

Position Summary: 

Center For Family Services is seeking an experienced bilingual medical professional for the Licensed Nurse position within the Juntos Program. The Nurse ensures the health and safety of all youth. They prepare and implements a comprehensive health plan to ensure that each resident's health needs are being met adequately and properly. The Nurse coordinates heath care for youth, dispenses medication, supervises, trains staff to dispense medication, ensuring documentation regarding medication, and meets healthcare requirements. 

 Duties and Responsibilities:

  • The Licensed Practical Nurse (LPN) coordinates, maintains, and updates the medication  management system under the direct supervision of the Registered Nurse Medical  Director, to allow for the administration of medication to UC in accordance with HHSC,  ORR, and DHHS policies and procedures. 
  • Ensures health and safety of all residents including provision of initial physical  examination next working day, scheduling follow-up physical examinations by a  physician within ten days and obtaining of health history of all new admissions. 
  • Communicates and collaborates with the RN to obtain technical assistance in the  implementation of medical services 
  • Completion of TAR's and TB supplemental. 
  • Collaborate with outside providers to attain UAC medical records, vaccination, lab  results, etc to assure all medical files are up to date in a timely fashion. 
  • Coordination of the provision of initial physicals by a licensed physician, 
  • Preparation and revision of medication logs, 
  • Timely reporting of medical incidents (e.g., SIRs, HHSC reports), 
  • Preparation of immunization records for County Health Department audits, Office of  Refugee Resettlement audits 
  • Ensures services are expedited by ensuring a physical screening is provided to UAC.
  • Maintains relationships with local providers, pharmacies, as well as with ORR and  DHHS. 
  • The LPN prepares and reviews statistical information for weekly, monthly, quarterly, and  annual reports. 
  • Arrange or partners with other health professionals regarding health and safety standards  for out of home childcare and childcare licensing requirements 
  • Follows disease-reporting requirements for care providers, immunizations for children  and injury prevention for children

Requirements:

  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license required. 
  • Registered Nurse (RN) license with a bachelor's degree in nursing (BSN) preferred. 
  • A bachelor's degree in a relevant field may be considered in lieu of an RN/BSN, based on  applicable program and position requirements. 
  • Minimum of one (1) year of professional experience in public health, healthcare, or a  related human services setting, with demonstrated experience in care coordination, health  administration, direct service delivery, or clinical care. 
  • Minimum of one (1) year of experience working with children and/or adolescents in a  child welfare, social services, healthcare, or other related service setting preferred. 
  • Must possess and maintain a valid driver's license in the state of residence. 
  • Must have at least three (3) years of licensed driving experience. 
  • Must successfully pass an initial motor vehicle record (MVR) check and maintain a  satisfactory driving record throughout employment. 
  • Must be able to meet agency and program requirements related to transportation and  travel, as applicable to the position.
