Center for Family Services is seeking a Community Case Manager for our Camden DREAMS Program. Camden DREAMS is looking for an experience Case Manager to provide direct services, conduct client visits, complete required documentation, and other reporting requirement pertaining to a caseload. The Community Case Manager serves Camden DREAMS community based young adults living independently. This position is part-time up to 28 hours per week and requires travel to Camden, NJ.

Duties & Responsibilities:
The Community Case Manager coordinates and provides individual and group living skills assessment and training for all residents and also monitors and performs individualized life skills instruction including ensuring the completion and updating of individualized life skills plan for each resident. This individual designs a case management plan for each youth assigned in caseload and updates the plan on a weekly basis. The Community Case Manager transports youth in caseload at the onset of a youth's placement and to various medical, recreational, educational, and employment appointments. The Community Case Manager advocates for youth and performs crisis intervention, counseling, youth supervision, and other related activities to meet 24-hour operational activities of the foster care program. The Community Case Manager meets regularly with supervisor, participates in continuing professional development opportunities, and contributes on Agency committees.
