Position Summary:

Center for Family Services is seeking an experienced individual for the Case Manager position within its STAR Program CC.  The Case Manager possesses the knowledge, skill, and experience necessary to competently perform case management activities while providing a comprehensive case management assessment of each individual in the program. The Case Manager is a key member of the program's team and provides services to ensure that individuals are both informed and able to access the full range of medical and treatment opportunities.

Duties and Responsibilities:

The STAR Case Manager develops an Integrated Family Case Plan that is person-centered, includes strategies for recovery, provides links to appropriate care, resources in the community, promotes the STAR individual's recovery to his/ her fullest potential by facilitating assessment of skills, and refers clients to help them access substance use disorder treatment if needed.  The Case Manager documents that each individual was offered a visit minimally one hour per week by a staff member and determines the type and frequency of services desired.  The Case Manager assists with transitioning individuals to another level of care to ensure continuity of care if needed, coordinate needed interventions, and services provided by multiple agencies.  The Case Manager uses engagement skills to motivate and encourage individuals to voluntarily access services that would assist them in maintaining a healthier lifestyle.  Also advocates on behalf of the individual, provides education to clients on HIV and Hepatitis, provides training to clients on the recognition of relapse triggers, and strategies to deal with avoiding a relapse.

REQUIREMENTS:

  • Bachelor's Degree, CADC required, LCADC and Master's Degree preferred
  • Three years' experience working with high need individuals involved with substance use disorders
  • Excellent driving skills and current driver's license with a driving record of at least three years 
  • Vehicle with current insurance and registration
