POSITION SUMMARY:

To work with the Gloucester County Department of Corrections, and the Salem County Correctional Facility Medical Staff, and Jail Social Worker to increase accessibility/access to Medication Assisted Treatment to eligible inmates who identifies with opioid use disorders. the community regardless of the length of stay within the correctional facility.

Duties and Responsibilities:

• To ensure appropriate referrals to community-based services 

• Providing linkages to ensure successful inmate continuation of MAT upon discharge

• Developing a WRAP plan, Wellness Recovery Action Plan to address the four domains of recovery; home, health, purpose, and community

• Work directly with Case Manager to discuss all treatment plans, and any barriers or supports 

• Peers will have continuous follow-up with client for 90 days

• All Peers will follow CFS policies, procedures, and trainings accordingly 

REQUIREMENTS: 

• High school diploma or equivalency

• Certified Peer Recovery Specialist Certification is required 

• Two years experience in the guiding principles of recovery that assist individuals to improve their health and wellness
