Job Type/Status: Full-Time
Duties and Responsibilities:
Delivers resources and information including funding and eligibility criteria and connection and diversion resource awareness for four-county SNJCoC goals and projects for Coordinated Assessment with a particular focus on Domestic Violence services and resources.
Uses iCarol cloud-based data base system to track work flow.
Makes appropriate referrals for program participants and referents as needed.
Maintains appropriate paperwork including program documentation.
Schedules and conducts HUD voucher centralized intake appointments with consumers and case managers. Delivers pre-screening and landlord readiness criteria to SNJCoC stakeholders.
Maintains a monthly schedule via CFS electronic calendar that ensures program level of service in accordance with licensing requirements and the needs of the program.
Meets with Program Director monthly for supervision and staff meetings.
Schedules intakes and vulnerability assessments as needed.
Tracks consumer progress toward housing goals and communicates with stakeholders on progress and follow up needs.
This position requires attention to detail in conducting centralized intake processes, and performs assessments and scoring, to place eligible candidates on voucher-based housing lists.
Acts as a liaison and makes direct referrals to local service programs that address housing needs, and makes a plan to set up candidates for success by providing resources around landlord readiness.
Delivers timeliness and fidelity to data entry and programmatic paperwork requirements, regular meetings with supervisor for individual and group supervision, staff and program development, and professional development.
Must be willing to flex time as needed to ensure program coverage on an as needed basis including some nights, weekends and holidays.
Builds and maintains relationships and collaborations with providers to promote the program and to provide supportive services.
Ensures continuum of services facilitated through Coordinated Assessment services.
REQUIREMENTS:
Minimum of a Bachelor's Degree in Social Work or related field, commensurate experience in working with homeless populations.
Three or more years of experience working with the program population.
