Position Summary:
Center For Family Services is seeking an experienced and professional individual for the Case Manager position within the SAFE Supportive Services in Affordable Housing Communities program. This program provides an array of supportive services to apartment residents in an effort to connect individuals to the resources that will benefit themselves and their families. The Case Manager provides case management services for residents of several Conifer Apartments in Camden, Gloucester, and Burlington Counties and coordinates workshops, social services, and training sessions for residents in the program.
Duties and Responsibilities:
The Case Manager organizes and coordinates a network of formal and informal activities, services and supports to optimize the well-being of a person, as well as coordinates and links individuals to community resources. This individual meets regularly with supervisor, completes documentation and other reporting requirements, participates in agency collaboration team meetings, and adheres to policies and procedures of CFS. The Case manager provides outreach and connects residents with the Case Management Services provided on-site and coordinates the planning and execution of a minimum of six on-site workshops/events that focus on an identified need of the resident community. This individual works with the Community Manager to develop an identified space on-site for information sharing regarding community resources and services, provides a monthly program report to the Apartment Manager, and conducts periodic surveys to further identify the needs of the population.
