Job Type/Status: Full Time

Salary: $52,000 annually

Duties and Responsibilities

• Complaint with funding source, contract and or program's Annex A
• Complete required documentation within established timeframes per Federal, State, County, and agency regulations, policy, and procedure.
• Provide case management: evidence-based curriculums, provide individual and/or family supportive counseling, and provide referrals, advocacy, and linkages.
• Providing a comprehensive case management assessment that addresses life domains such as housing, finances, transportation, legal services, vocational, health care, and family strengths/needs.
• Provide case management services to ensure that adolescent and family members are both informed about and able to access the full range of medical and treatment opportunities or other support services, such as legal, financial, educational, vocational, or employment counseling, that may be available to them at the time the individual chooses to use them.
• Participate in the development of service plans, comprehensive assessments, discharge planning, and case conferences. Develop an Integrated Family Case Plan that is person-centered and includes strategies for recovery. The plan shall identify priorities, desired outcomes and the strategies and resources to be used in obtaining outcomes based on the case management assessment.
• Coordinate interventions and services provided by multiple agencies, including within CFS programs.
• Assist and engage the individual with transitioning to another level of care (outpatient substance use disorder treatment) to ensure continuity of care within the CFS agency.
• Coordinate needed interventions and services provided by multiple agencies.
• Coordinate with community partners such as schools to provide updates on client's progress.
• Use engagement skills to motivate and encourage individuals to voluntarily access services that would assist them in maintaining a healthy lifestyle.
• Appropriately utilizes supervision process and agency supports.
• Participate in team meetings regarding the individual's behaviors and ongoing service needs.
• Collects statistics and completes written reports as required.
• May provide specialized activities and functions specific to program.
• Advocate on behalf of the individual.
• Provide education to clients on communicable diseases.
• Some nights and weekends may be required.
• Some transportation of clients in an agency vehicle may be required.

Qualifications

• High School Diploma, Bachelor of Social Work degree or comparable Bachelor of Arts degree in Human Services, Sociology/Psychology from an accredited institution.
• Experience in case management preferred.
