POSITION SUMMARY:

As Program Supervisor of a core services team, provides direction to the team in a wellness and recovery-oriented manner; also provides direct service to consumers who have a serious and persistent mental illness and are enrolled in the IOC program. 

Duties and Responsibilities:

  • Creates and maintains a wellness and recovery-oriented environment by using person-centered planning, language and documentation, and by insuring that services provided promote wellness and recovery outcomes.

  • Insures direction and evaluation of IOC team members and participates in the hiring and termination process of IOC staff; as needed, provides education and technical assistance to team members regarding recovery and wellness; works with the team to develop relevant strategies, such as Wellness Recovery Action Plans (WRAPs), instilling hope, promoting choice and empowerment, and developing social and spiritual supports; serves as a resource for the team for wellness and recovery strategies.

  • Insures that relevant data is collected for contract monitoring, quality improvement, and other purposes as needed; insures that data is used to improve services provided; 

  • Insures coordination and integration of IOC services with other Center for Family Services programs and other community programs/agencies in a manner that benefits consumers served.

  • Insures coordination and integration of IOC services with the WC Superior Court, the WC Adjuster's Office, and with attorneys representing consumers and the County at commitment hearings in a manner that benefits consumers served; acts as the liaison to the Court, the Adjustor's office and the attorneys involved in the judicial process of commitment.

  • Provides 24-hour on-call coverage on a rotating schedule.

  • Represents IOC at Court proceedings, care coordination meetings, and other local and/or state-wide meetings as needed.

  • Insures that staff is oriented to and operates according to established agency and program procedures.

  • Actively participates in supervision. 

  • For self and IOC team members, is responsible for insuring continuing educational development and for meeting licensure requirements as applicable.

  • Provides direct service to IOC consumers and their family members; works with the consumer and family to assess the appropriateness for the IOC  services works with the consumer, family, IOC psychiatrist and the referring entity to develop an initial treatment plan; works with consumer and family members to develop and implement a Wellness Recovery Action Plan (WRAP) and to develop treatment/WRAP plan adherence supports using consumer peer specialists as indicated; conveys hopefulness toward people served and their families.

  • Works with consumer, family and other service providers as indicated to link consumer to clinical and other services as identified in the treatment plan, the WRAP and a Psychiatric Advance Directive. 

  • Provides outreach to consumers/family members and encourages consumers to participate in mental health services; involves family members, friends, consumer peer specialists and others concerned with the welfare of the consumer to achieve consumer cooperation. Outreach is provided in the consumer's home, in other community agencies or elsewhere in the community as per the consumer's choice. 

  • Provides frequent and ongoing monitoring and assessment of IOC consumers to determine clinical progress and to determine imminent dangerousness to self, others or property; provides crisis intervention as needed, and involves the Designated Screening Service when the consumer requires screening for involuntary inpatient commitment.

  • Insures/provides transportation for the consumer to facilitate attendance at all scheduled Court hearings, mental health evaluations, participation in mental health services, and to insure linkage with key community services (e.g., Temporary Assistance, health services). 

  • Works with the consumer, family members, and other members of the IOC team to assess readiness for discharge from IOC. Makes recommendations to the Court regarding consumer's clinical progress and the need for continued involuntary outpatient commitment; provides testimony to the Court during scheduled commitment hearings.

  • Participates in quality improvement activities within the program and participates in agency-wide quality improvement activities, as directed.

  • Represents IOC at Court proceedings, care coordination meetings, and other local and/or state-wide meetings as needed.

  • Completes/submits all reports and other documentation in a timely manner;

  • Other duties as assigned. 


REQUIREMENTS: 

Master's degree in Social Work or an allied field of study, licensed in NJ to provide clinical services, 2 years of experience working with the serious and persistent mentally ill population including the dually diagnosed (Co-occurring) population, valid driver's license with acceptable driving record. LCSW with supervisor's credentials and CADC preferred.
